Warfarin dose and INR related to genotypes of CYP2C9 and VKORC1 in patients with myocardial infarction
Background: Warfarin treatment has a narrow therapeutic range, requiring meticulous monitoring and dosage titration. Individual dosage requirement has recently partly been explained by genetic variation of the warfarin metabolizing enzyme CYP2C9 and the Vitamin K-activating enzyme VKORC1. In the WARIS-II study, comparing three different antithrombotic regimens after myocardial infarction, warfarin treatment reduced thrombotic events, but was associated with more frequent bleeding than use of acetylsalisylic acid (ASA) alone.AimsThe primary aim of the present study was to investigate the relation between genotypes of CYP2C9 and VKORC1 and warfarin maintenance dose in myocardial infarction. The secondary aim was to relate the genotypes to international normalized ratio (INR). Methods: Genotyping was performed in 212 myocardial infarction patients from the WARIS-II study by robotic isolation of DNA from EDTA whole blood (MagNa Pure LC) before PCR amplification (LightCycler) and melting point analysis. Results: The 420 C>T substitution of CYP2C9*2, the 1075 A>C substitution of CYP2C9*3 and the 1173 C>T substitution of VKORC1 had minor allele frequencies of, 11.3%, 5.7% and 36.6% respectively. Warfarin weekly dose varied between 17 mg and 74 mg among the patients. INR did not vary between genotypes. Warfarin dosage requirement was significantly associated with CYP2C9 and VKORC1 genotypes, treatment group and age. The VKORC1 genotype contributed 24.5% to the interindividual variation in warfarin dosage, whereas the combined CYP2C9 genotypes were only responsible for 7.2% of the dose variation. Conclusion: CYP2C9 and VKORC1 genotype frequencies in myocardial infarction patients appear similar to other patient groups and have similar impact on warfarin maintenance dose.

Effects of rosiglitazone on contralateral iliac artery after vascular injury in hypercholesterolemic rabbits
Background: The objective was to evaluate the effects of rosiglitazone on iliac arteries of hypercholesterolemic rabbits undergoing balloon catheter injury in the contralateral iliac arteries. Methods: White male rabbits were fed a hypercholesterolemic diet for 6 weeks and divided into two groups as follows: rosiglitazone group, 14 rabbits treated with rosiglitazone (3 mg/Kg body weight/day) during 6 weeks; and control group, 18 rabbits without rosiglitazone treatment. All animals underwent balloon catheter injury of the right iliac artery on the fourteenth day of the experiment. Results: There was no significant difference in intima/media layer area ratio between the control group and the rosiglitazone group. Rosiglitazone did not reduce the probability of lesions types I, II, or III (72.73% vs. 92.31%; p = 0.30) and types IV or V (27.27% vs. 7.69%; p = 0.30). There were no differences in the extent of collagen type I and III deposition or in the percentage of animals with macrophages in the intima layer. The percentage of rabbits with smooth muscle cells in the intima layer was higher in rosiglitazone group (p = 0.011). Conclusion: These findings demonstrate that rosiglitazone given for 6 weeks did not prevent atherogenesis at a vessel distant from the injury site.

Therefore, the iatrogenic death rate dwarfs the annual automobile accident mortality rate of 45,000 and accounts for more deaths than all other accidents combined.” These statistics have become a lot worse since 1995. Unless you require an emergency treatment, it is better to avoid hospitals altogether. Many hospitals today may pose a major risk to your health for the following reasons:* They are filled with infection-causing bacteria that cannot be found anywhere else. Hospitals, which often house very large numbers of sick people, are the ideal breeding environment for the sometimes deadly bugs. Hospital patients generally have a lower level of immunity and offer little or no resistance to them. Many of the microbes are passed on to the patients through the cooling towers, air conditioning and heating systems in hospitals. The hospital staff, due to constant exposure to the bugs, are fairly immune to them, but may pass them on to patients by touching them or their food, bedding, clothing, or medications.* Contrary to common belief, hospitals are among the most contaminated places in the world. In fact, it is virtually impossible to keep hospitals spotlessly clean, and it does not take much dirt to become a breeding place for billions of deadly infectious bacteria.* Doctors can be the worst transmitters of disease in hospitals. Most doctors do not wash their hands except before an operation, when they wear sterilized gloves and gowns anyway. They may sometimes touch many dozens of patients within several hours, one after the other, without washing their hands even once. Even the doctor’s white gown is not as clean as it looks. It is only clean if it is washed every single day, which rarely happens. When it is washed, it comes into contact with the dirty laundry from the operating room, bed covers, pillowcases, etc. Many extremely harmful bugs survive the washing machine and dryer.* Bed sheets may be clean, but mattresses and pillows are not. The chance of being infected by bugs living in them is 1 in 20.* Fifty percent of all infections in hospitals occur because of the patient’s contact with non-sterile medical instruments such as catheters and intravenous infusion installations. Before they were in common use, such infections occurred only very rarely.* In the United States, over 90,000 people a year die from hospital-acquired infections. This figure does not account for those who are considered to be dying, or are already weakened by an operation. Yet they, too, are killed by a hospital-acquired infection.* A 1,500-page report of a 3-year study on the causes of death in American hospitals revealed that a further “300,000 Americans die each year in hospitals as a result of medical negligence.”* The most dangerous place in a hospital is the maternity ward because infants have not gained immunity against any disease-causing agents. The most vulnerable babies are these who are deprived of the antibodies contained in breast milk.* A hospital patient may receive up to 12 different kinds of medication, all of which produce side effects that can lead to serious complications and even death.* Many studies have shown that between 25 and 50 percent of the long-term patients staying in U.S. and U.K. hospitals are suffering from malnutrition due to a poor hospital diet. Malnutrition was found to be the major cause of death among older people in hospitals. An undernourished body is hardly able to defend itself against any type of illness. Add the toxic side effects of the drugs, the presence of deadly bugs, as well as the stress and anxiety that accompany an illness and a stay in a hospital, and a poorly nourished elderly person has very little chance of surviving.* A spot check of 105 U.S. hospitals conducted by the American government showed that 69 of them had violated basic laws and rules. The commission in charge of granting licenses to hospitals (JCAH), however, refused to close them down.* Most deliveries today take place in the operation theaters of hospitals, which when compared with home deliveries, increases the infant’s risk of injury during delivery by six times, of getting stuck in the mother’s birth canal by eight times, of requiring resuscitation techniques by four times, of becoming infected by four times, and of developing chronic physical problems by thirty times. In addition, a mother is three times as likely to hemorrhage if she gives birth in a hospital.* More than 3,000 hospital patients in the U.S. undergo wrong-side surgery each year.Given these and other major health risks linked with a stay in the hospital, it can be said that hospitals are among the most dangerous places in the world. Microsoft Opens Windows 7 Blog for Discussions
Microsoft has launched a new blog page for discussions with customers and partners about the next Windows operating system. “We strongly believe that success for Windows 7 includes an open and honest, and two-way, discussion,” said Windows 7 development engineers Jon DeVaan and Steven Sinofsky. For a while now, Microsoft has said it would deliver Windows 7 approximately three years after the January 2007 launch of Windows Vista. “The product is tracking very, very well,” said Senior Vice President Bill Veghte, who runs Microsoft’s Windows business. “And we are looking good relative to our commitment,” he told analysts last month. To prep developers on the new OS, Microsoft will hold two major Windows events. “The Professional Developers Conference on October 27 and the Windows Hardware Engineering Conference the following week represent the first venues where we will provide in-depth technical information about Windows 7,” DeVaan and Sinofsky said. Vista Under Fire Sales of licenses for Windows Vista surpassed the 180 million in the second quarter, according to Colleen Healy, general manager of investor relations at Microsoft. “And Windows Vista had driven client revenue to an average growth rate of 16 percent since it became generally available,” she said during a conference call with analysts last month. However, Vista has come under fire because of its failure to work well with other software applications and hardware. Adoption rates in enterprises have also lagged analysts’ expectations. With the launch of Windows 7, Microsoft will have its first major opportunity to improve industry perceptions about its operating system. “The biggest problem Vista has today is in perception — a perceived lack of value and perceived instability,” said Michael Silver, a research director at Gartner Client Computing. iPhone 3G Problems Appear Centered on Big Cities
Reports continue to flood in from U.S. iPhone 3G users about problems with 3G connections and speeds. The problems tend to come from large metropolitan areas, but one informal survey points to the San Francisco Bay Area and Los Angeles. The problem, BusinessWeek reported Thursday, may be faulty firmware in a communications chip supplied by Munich-based Infineon. BusinessWeek said its confidential sources backed a report released earlier this week by Nomura Securities analyst Richard Windsor that pointed to the firmware. Patch May Be Imminent An Infineon spokesperson declined to address the iPhone issue directly, but defended Infineon’s technology, saying it works fine in other 3G phones. “Our 3G chips are, for example, used in Samsung handsets and we are not aware of such problems there,” he said. Apple is reportedly working on a firmware upgrade to fix the problem. USA Today reported that the fix could be available via iTunes as early as next week, but Apple has not commented. Tim Bajarin, principal analyst with Creative Strategies, said in an e-mail, “Historically, Apple does not respond to something like this automatically just because there are reports of problems. If there is merit to the issue, they work overtime to fix it and usually, at the point of the fix, that is when they respond.” Bajarin expects Apple to eventually make things right with its 3G customers. “Apple prides itself on its customer-service reputation and I would be surprised if they don’t go the extra mile to make sure anything that impacts that image will be corrected as fast as possible,” he said. U.S. sales of video-game consoles and software jumped 28 percent last month compared to last year, according to market researcher NPD Group. Nintendo’s DS portable and Wii remained the most popular systems. For hardware, that means nearly $450 million in July purchases, a 17 percent increase compared to the same time last year. The top-selling DS moved about 608,000 units, with the Wii second at about 555,000. PS 2 Still Popular The next four places showed small differences, with third-place Sony’s PlayStation 3 at 225,000, its PlayStation Portable in fourth with 222,000, and Microsoft’s Xbox 360 fifth with 205,000. The PlayStation 2, now entering late middle age eight years after its release, still sold about 155,000 for sixth place. In total sales, the Wii is in first place, with about 13.5 million sold in North America and about 31 million worldwide. The Xbox 360 is in second place, with 12 million U.S. sales and 20 million worldwide, and the PS3 takes third with 5.5 million U.S. and 15 million worldwide. Thrombosis is a recognised complication of inflammatory bowel disease (IBD), in particular venous thrombosis. Arterial thrombosis, especially stroke is rare. There is a paucity of information regarding stroke in IBD and its management. The authors describe two cases of stroke in patients with IBD during periods of increased disease activity. The literature regarding this devastating complication and the procoagulant state that exists in IBD are reviewed.

Examining warfarin underutilization rates in patients with atrial fibrillation: Detailed chart review essential to capture contraindications to warfarin therapy
IntroductionAtrial fibrillation affects an estimated 2.5 million Americans and incurs an average annual stroke risk of 4.5% per year. Despite warfarin reducing stroke risk by approximately 66%, prior studies show warfarin usage rates to be about 50%. However, the methods that define warfarin as “inappropriate underutilization” might not be sensitive enough to pick up relative contraindications. We assessed the inappropriate underutilization of warfarin in atrial fibrillation patients at our hospital by abstracting individual patient charts. Methods: medical records were reviewed to determine stroke risk factors, warfarin use, and documented contraindications to warfarin use in 364 consecutive patients with atrial fibrillation. Results: Amongst 364 atrial fibrillation patients, 54.6% received warfarin anticoagulation. Overall, 29.5 % of patients had documented reasons for not prescribing warfarin. Primary reasons listed by treating physicians included: gastrointestinal bleed 10.7%, secondary/transient atrial fibrillation 8.2%, and fall risk 6.3%. Only 7.1% of the patients had no documented reasons for the lack of warfarin use. Conclusion: Consistent with previous reports, 45.4% of patients in this atrial fibrillation cohort were not prescribed warfarin. However, after reviewing medical charts for documented reasons why warfarin was not used, the inappropriate underutilization rate was only 7.1%. These findings suggest that studies utilizing administrative database and ICD-9 CM coding might overestimate warfarin underutilization.
